Pick Up of Donated DME

  1. Durable home medical equipment (DME) acquired through REquipment is for personal use only and may not be sold.
  2. REquipment devices are not provided on loan. Recipients keep the equipment for as long as it is needed and agree to donate it back to the program if they are no longer using it. Returned devices are considered new donations and subject to all program policies at the time of consideration.
  3. REquipment provides no more than one power wheelchair, manual wheelchair, or scooter to a single device recipient within a 12-month period.
  4. Recipients are responsible for learning to use their equipment properly and for acquiring manuals and operating instructions when REquipment cannot provide them.
  5. Recipients accept REquipment DME “as is.” REquipment DME is not evaluated by a licensed technician and is not covered under a manufacturer warranty.
  6. REquipment delivers DME only to accessible locations. REquipment drivers are not permitted to carry heavy items (e.g., sling lifts, power wheelchairs, scooters) up or down stairs.
  7. Upon inspection and acceptance of REquipment DME, recipients sign a delivery receipt. Recipients assume responsibility for any problems encountered while using their device(s) and all future maintenance and repair needs.
