Wheelchair Wash Waiver

Participant Release, Waiver of Liability, and Photo Consent

Acknowledgment and Assumption of Risk

I voluntarily choose to participate in the REquipment Wheelchair Wash Event. I understand that volunteers will assist in washing and cleaning my wheelchair or mobility device using reasonable care.

I acknowledge that there are inherent risks associated with participating in this event, including, but not limited to, minor scratches, cosmetic damage, water exposure, or unforeseen equipment issues. I understand that REquipment and its volunteers cannot guarantee that my mobility device will be free from damage.

Release of Liability

In consideration of being allowed to participate in this event, I hereby release, waive, and discharge REquipment, its officers, directors, employees, volunteers, sponsors, partners, and agents from any and all claims, liabilities, demands, damages, or causes of action arising out of or related to my participation in the event or the cleaning of my mobility device, except to the extent caused by gross negligence or willful misconduct.

I certify that my wheelchair or mobility device is in safe operating condition and that I am responsible for notifying volunteers of any areas that should not be cleaned or exposed to water.

No Repair Services

I understand that this event is intended solely to provide exterior cleaning of mobility equipment. It is not a repair, maintenance, inspection, or safety evaluation service. REquipment makes no representations regarding the condition or operation of my equipment before or after the cleaning.
